Referring to Blackbird Counselling

Blackbird Counselling accepts referrals from various avenues. Our most common referrals are GP Mental Health Care Plans, Non Directive Pregnancy Counselling, and Private/Community Referrals.

Blackbird Counselling does not offer Bulk-Billed Sessions.

HOW DO I REFER TO BLACKBIRD COUNSELLING?

We recommend utilising our GP and community referral forms to ensure we receive all the information we require. Please send any additional documentation available (eg: GPMHCP or GP referral letter).

Referrals can be sent via email or fax. We also use Medical Objects.

WHAT HAPPENS ONCE I HAVE REFERRED MY CLIENT TO YOUR SERVICES?

Once we receive a referral, we will make contact with the client and book in their initial session. Once the client has attended their first session, the clinician will write a letter to the referrer. Our clinicians uphold a professional relationship with referrers and will always make contact if there are any concerns.

General Practitioner (GP) Referrals:

Blackbird Counselling welcomes GPMHCP referrals. DOES THE REFERRAL MEET MEDICARE REQUIREMENTS?

Blackbird Counseling requires all GPMHCP referrals to meet the Medicare requirements. To make it easier for you, we have listed the requirements below in which MUST be included in your referral:

  • Patients name;

  • Patient’s date of birth;

  • Patient's residential address;

  • Patient’s contact phone number;

  • A list of the patient’s current medications;

  • An eligible diagnosis;

  • The number of sessions the patient is being referred for:

    • 6 Initial Sessions under a MBS Mental Health Care Plan

    • 4 Additional sessions under a MBS Mental Health Care Plan (to make up the initial 10)

    • 10 sessions under the Additional MBS COVID sessions (if the patient has already completed the initial 10 sessions).

    • 3 Non-Directive Pregnancy Counselling Sessions

    • Other:

  • Referring GP name;

  • Referring GP practice;

  • Referring GP provider number;

  • Referring GP signature.

To ensure that we do not have to amend any referrals, we have constructed a printable referral form template with these requirements outlined. Alternatively, this information can be in the form of a signed and dated referral letter.

Other Clinic Referrals:

Blackbird Counselling welcomes referrals from members of the community wanting to refer themselves or from other professions wanting to refer people to our services.
